“we” or “us” refers to Smart Clinic Forms
“customer subscription” or “customer’s subscription” refers to an online subscription setup to use any of the Smart Clinic Forms’ online management tools.
“customer” refers to any person or entity that has a valid Customer subscription with Smart Clinic Forms.
“user” or “you” refers to any person or entity that has a valid login to a Customer subscription with Smart Clinic Forms.
General
This privacy policy covers how Smart Clinic Forms treats information it collects and receives about its customers and users. Anyone who visits the Smart Clinic Forms’ website or uses/accesses any of Smart Clinic Forms’ applications shall be deemed to have agreed to the terms of this privacy policy. We use your personal information for providing and improving the service. We will not use or share your information with anyone except as described in this Privacy Policy.
INFORMATION WE COLLECT AND HOW WE USE IT
Contact information: While using our service, we may ask you to provide us with certain personal information that can be used to contact or identify you. Personal information may include, but is not limited to, your email address, name, phone number, postal address, other information. We collect this information for the purpose of providing the service, identifying and communicating with you, responding to your requests/inquiries, servicing your purchase orders, and improving our services.

Please note that some communications (such as service messages, subscription notifications, billing information) are considered transactional and necessary for subscription management and you cannot opt out of these messages unless you request to deactivate your Smart Clinic Forms’ subscription.
